Overview

The Clinical Pharmacist will provide advanced collaborative pharmaceutical care to patients on their units.  The pharmacist will work in collaboration with providers, nurses, and pharmacy leadership to recommend and monitor the medication utilization, adverse drug reactions, and therapeutic regimens of patients.

 

The Clinical Pharmacist shall serve as a clinical leader in their area and act as a liaison between the pharmacy staff, the Clinical Pharmacy Manager, and the NMC Pharmacy Manager. The Clinical Pharmacist shall, according to standard procedure: 1.) Facilitate the delivery of clinical services and the resolution of unresolved problems related to the clinical services in their area of expertise; 2.) Act as the Pharmacy Department representative for hospital wide initiatives in their subject area; 3) Be responsible for assuring the accurate and timely delivery of the drug information and other clinical services to patients and other health care providers; 4.) Enhance the delivery of the clinical pharmacy services through the development of a personal clinical practice, development of the clinical abilities of the staff, and education of the pharmacy students/residents.

Responsibilities

  1. Clinical Practice
  1. Develop and maintain an area of clinical expertise in practice area. Staffs in a patient care area providing order entry and verification and clinical pharmacy services.
  2. Routinely rounds with multidisciplinary team and contributes to care of patients in aspects related to medication usage.
  3. Maintains clinical skills knowledge to cross cover for other areas.
  4. Supports clinical services/programs including Pharmacokinetics, Automatic Renal Dosing, Antimicrobial Stewardship, IV to PO Conversion Program, VTE prophylaxis and pharmacist’s order entry and intervention assessment.
  5. Monitors patient drug therapy for patients in the clinical practice area to ensure efficacious, safe, and cost effective drug therapy.
  6. Promotes rational and economic drug use through professional interactions with the medical staff and medical staff committees.
  7. Oversees the medication history intake program at Northport, completing any training, education, or competency assessment related to the maintenance or expansion of the program.
  8. Leads the Leap Frog Medication Reconciliation process for data collection, reporting, and follow-up.
  1. Performance Improvement
  1. Chairs or participates as an active member on hospital and department committees. (i.e. P & T, Critical Care, Stroke, Trauma, Pain Management, Critical Care, Code Blue, Sepsis, Infection Prevention, Heart Failure, Residency Advisory Committee, etc.)
  2. Monitors the medication use process on a routine basis and reports inconsistencies to improve mediation safety within the hospital. Actively participates in at least 1 Core Measures or other performance improvement activity each year.
  3. Documents improvements in patient outcomes and cost containment efforts in the department’s clinical intervention system. 
  4. Routinely reports Medication Events and proposes ideas to decrease events including development and revision and implementation of order sets.
  5. Participates in other performance improvement activities, as assigned by the Clinical Pharmacy Manager or other manager.
  6. Assumes responsibilities of the Team Leader or Pharmacy Manager, as assigned, in his/her absence.
  1. Education and Training
  1. Oversees all aspects of maintaining and supporting clinical services/programs, including staff development and competency assessment of all areas of expertise. Serves as an expert resource to staff.
  2. Works with management to develop competency areas and checklists and trains new pharmacists in those areas.
  3. Provides drug information to physicians, staff, and co-workers through direct contact, committee participation and quarterly staff educational programs
  4. Presents at least 2 Clinical Updates, continuing education programs or poster presentations at local, state, or national meetings each year.
  5. Serves as a resource for residency research, student research or conducts at least one research project (MUE acceptable) each year.
  6. Maintains a clinical pharmacy rotation for students/residents. Participates in all aspects of student or residency training for area of expertise.

Qualifications

Must hold a current license to practice pharmacy in the State of Alabama. Must be an Alabama pharmacy preceptor or eligible. The candidate must have developed an area of personal clinical practice, contributing regularly to patient care, and has exhibited a commitment to maintain a level of practice well beyond that required of staff practitioners.

 

This position requires a:

  • S. or PharmD. Degree required, with
    • Completion of an ASHP Accredited PGY1 Residency and 2 years’ minimum hospital experience or
    • Minimum of 5 years’ relevant hospital staff experience or
    • Completion of an ASHP Accredited PGY1 Residency and a PGY2 Residency in relevant area (such as Internal Medicine, Cardiology, Critical Care, Emergency Medicine, Infectious Disease, Pharmacotherapy)
    • BPS certification in area of expertise within 12 months of employment
  • Experience precepting pharmacy students and/or residents or training pharmacists

 

Membership and active participation in professional pharmacy organizations is expected.  Leadership experience is preferred.  Work experience on multidisciplinary teams and participating on Medical Staff Committees is preferred.  Must possess effective communication and supervisory skills.  Must be able to read, write legibly, speak and comprehend English.

 

Preferred skills-

  • BPS certification BCPS), or ability to obtain certification within 12 months of employment is preferred.
