Overview

Nurse Practitioner in collaborative practice with physician(s) provides primary care services to patients in Primary Care Clinic.  The NP will function autonomously in the clinics and will use his or her independent judgment along with the advice of the collaborating physician, as needed, to provide health care services.  

Responsibilities

 

  1. Performs primary care services including assessment and diagnostic services including diagnosis of patient illness, through clinical evaluations and ordering diagnostic test as indicated, providing necessary treatment and follow up care.
  1. Determines treatment plan(s) in collaboration with the supervising physician.
  2. Documents all patient encounters, interventions, and actions in a medically appropriate manner in the patient’s medical record.
  3. Performs a wide range of medical procedures under physician’s collaboration agreement.
  4. Responds to daily telephone requests from patients. 
  5. Writes appropriate prescriptions and administers medications in accordance ABN Scope of Practice and the collaborating agreement and treatment protocol established by the supervising physician. 
  6. Orders and interprets daily laboratory and radiology examinations. 
  7. Teaches and counsels patients and/or family members regarding educational needs appropriate to diagnosis and follow-up care.
  8. When indicated, coordinates referral of patients to different levels of care or to other specialties.
  9. Participates in the development, implementation, and evaluation of practice management guidelines.
  10. Participates in Performance Improvement Program and Quality Initiatives.
  11. Supports adherence to regulatory guidelines ascribed by State and Federal regulators.
  12. Maintain patient privacy in all matters including written medical records and computer records
  13. Maintains Corporate Compliance, HIPAA, Security and Red Flag Alert Plans/Procedures/Staff Training for the clinic in alignment with the DCH Standards/Policies/Procedures

  • Requires use of electronic mail, time and attendance software, learning management software and intranet.
  • Must adhere to all DCH Health System policies and procedures.
  • All other duties as assigned.

Qualifications

  1. Master’s or higher degree in nursing and a graduate of an approved Nurse Practitioner program
  2. Current Certified AL Registered Nurse Practitioner Licensure in good standing with Alabama Board of Nursing
  3. Prior experience working as a CRNP in a Primary Care or Urgent Care practice preferred
  4. Excellent verbal communication skills to interact with patients, coworkers and other visitors.
  5. Must be able to read, write legibly, speak, and comprehend English.

 

WORKING CONDITIONS

 

Ability to use medical supplies and equipment necessary to carry out the duties listed above.

Work is in a fast paced ambulatory setting.  Must be able to work independently.  Work requires walking, standing, sitting, lifting, reaching, bending, pushing, and pulling.  Must be able to lift and support weight of 75 pounds in handling patients and medical equipment and supplies.  Must speak English fluently and in understandable terms.  Must possess an intact sense of sight, hearing, smell, touching and finger dexterity.  Must have the ability to concentrate.  Must be able to respond quickly to changes in patient and/or clinic conditions.  Possible exposure to communicable diseases.

Will limit access to protected health information (PHI) to the information reasonably necessary to do the job.  Will share information only on a need to know basis for work purposes.  Access to verbal, written and electronic PHI for this job has been determined based on job level and job responsibility within the organization.  Computerized access to PHI for this job has been determined as described above and is controlled via user ID and password.

Physical presence onsite is essential.  Hearing and vision must be normal or corrected to within normal range.  Able to perform the duties with or without reasonable accommodation
