Overview

Retrieves and files medical records for the purpose of patient care and other requests as appropriate following established hospital and department policies and procedures.  Analyzes and reviews record for completeness and routes appropriately.

Responsibilities

  1. Picks up discharged inpatient and outpatient charts from all units daily Monday through Friday and maintains accountability for discharged patient records.
  2. Prep and scan all charts and record documents retrieved from all units daily.
  3. Performs indexing/quality review on all charts and record documents and uploads in Expanse
  4. Perform release of information functions according to hospital and department policies and procedures.
  5. Performs data entry and submission of statistic information following required guidelines.

  • Performs compliance requirements as outlined in the Employee Handbook
  • Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
  • Requires use of electronic mail, time and attendance software, learning management software and intranet.
  • Must adhere to all DCH Health System policies and procedures.
  • All other duties as assigned.

Qualifications

MINIMUM KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED

 

Must have good communication and organizational skills.   High School diploma or equivalent required. Must be able to read, write legibly, speak and comprehend English.
