Overview

Provides direct nursing care to the residents and supervises the day-to-day nursing activities performed by nursing staff in accordance with current federal and state regulations. Acts as a liaison between the facility and the community coordinating the admission process between the facility direct care staff, the family, the hospital and physicians.

Responsibilities

  • Performs compliance requirements as outlined in the Employee Handbook
  • Manages front line operations of area of accountability related to meeting/exceeding departmental goals, 5 keys of excellence, DCH Mission and Vision.
  • Ensures that Level I and II information and other preadmission requirements are completed prior to admission.
  • Communicates with administration; nursing and financial personnel to ensure requirements for admission are met.
  • Communicates admission information with interdisciplinary team and coordinate admission meetings with family.
  • Enters admission analysis information in computer and coordinates with other departments.
  • Coordinates room availability with social services and verify acceptability with families.
  • Coordinates total nursing care for assigned residents. Assesses resident conditions, including physical, behavioral, psychosocial, developmental and age specific plans according to identified problems in a cost effective manner.
  • Directs LPN's and CNA's on a daily basis, including scheduling, supervising, evaluating, counseling, and daily operations.
  • Educates, supervises and counsels the residents/family and caregivers regarding long term care procedures, residents' rights, and other care needs as appropriate to the resident's condition.
  • Represents the facility at other organizations and special functions in a community educational capacity.
  • Develops and prepares the individual's plan of care, utilizing RAI guidelines in accordance with state and federal regulations and follows up with new admissions to ensure needs are being met. Performs duties based on established standards of practice
  • Participates in weekend staffing coverage for the unit.

  • Performs compliance requirements as outlined in the Employee Handbook
  • Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
  • Requires use of electronic mail, time and attendance software, learning management software and intranet.
  • Must adhere to all DCH Health System policies and procedures.
  • All other duties as assigned.

Qualifications

Must be a Registered Nurse in good standing and currently licensed by the state of Alabama. Supervisory experience preferred. Current CPR certification required. Must be able to read, write, speak, and comprehend English. Prior experience in formal presentations and/or marketing preferred. Computer skills/general knowledge of computer systems for retrieval and input of data required.

Demonstrates communication skills for all population served. Must hold a current Alabama driver’s license and be insurable under the DCH insurance policies. Must be able to travel to other facilities and provide own transportation. Must have the ability to evaluate and interpret information and make competent and independent decisions as well as handle stressful situations and multitask. Positive attitude toward the elderly and those with impaired functioning.
