Overview

The Clinical Manager creates a positive culture of accountability through instruction, mentoring, coaching, and field supervision. They are responsible for supervising patient care, as well as implementing best practice and industry standards linked to agency quality and financial performance. The Home Health Clinical Manager is available at all times during operating hours (or appoints a similarly qualified alternative) and participates in all activities relevant to services furnished. They fulfill job related requirements timely.

Responsibilities

LEADERSHIP

  1. Supports the DCH Health System Mission, Vision and Strategic objectives.
  2. Serves as a Home Care Leader displaying sound managerial judgment, exemplary customer service behaviors, excellent team building and people skills at all times.
  3. Manages home health operations meeting/exceeding departmental goals.

 

PATIENT CARE MANAGEMENT

  1. Manages the day-to-day provision of appropriate care to patients utilizing best practice and industry standards resulting in high quality and financially responsible care.
  2. Oversees the scheduling of patients and assures they are seen timely.
  3. Leads performance Improvement activities.
  4. Responsible for the coordination of interdisciplinary team care, assuring care is appropriate and that each patient is assigned a primary care managing nurse or therapist.
  5. Active participant in patient care meetings.
  6. Ensures documentation is completed in accordance with agency policy.
  7. Serves as a resource to staff.
  8. Participates in the development of standards which ensure safe and therapeutically effective services to patients and families. Assures standards are followed.
  9. Observes confidentiality and safeguards all patient related information.
  10. Ensures all necessary supplies and equipment are available.
  11. Monitors equipment for appropriate use.
  12. Addresses, documents, and resolves patient concerns
  13. Develops a cooperative relationship and communicates effectively and professionally with physicians and referral sources.
  14. Investigates, documents, and reports problems relating to patient care or conditions which might harm the patient and/or employee well-being.
  15. Immediately reports any accident, incident, lost articles or unusual occurrence to the director.
  16. Attends pertinent continuing education programs other than routine in-services and shares information with staff.
  17. As a member of the Home Health Leadership Team participates in the Senior RN On-call rotation.
  18. Available to share on-call with nursing as needed.
  19. Available to perform any type of nursing visit as dictated by the needs of the department.

 

STAFF DEVELOPMENT

 Assists in recruiting and hiring.

  1. Coordinates orientation and training for employees, assuring all assigned staff have the educational and training preparation to provide care. This may include preparation for certification.
  2. Performs home visit evaluations, documents and reports findings, and develops time limited improvement plans.
  3. Manages employee performance. Responsible for assigned staff performance evaluations.
  4. Executes progressive discipline up to and including termination.

 

OPERATIONAL/ADMINISTRATIVE

  1. Provides time keeping oversight for assigned staff.

 

DCH Standards:

  • Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
  • Performs compliance requirements as outlined in the Employee Handbook
  • Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
  • Requires use of electronic mail, time and attendance software, learning management software and intranet.
  • Performs essential job functions in a manner that ensures the safety of patients, visitors, and employees.
  • Identifies and reduces unsafe practices that may result in harm to patients, visitors, and employees.
  • Recognizes and takes appropriate action to reduce risks and hazards to promote safety for patients, visitors, and employees.
  • Must adhere to all DCH Health System policies and procedures.
  • All other duties as assigned.

 

MINIMUM KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED

 

Graduate of a school of nursing, (BSN preferred). Current Alabama license  Must be knowledgeable in federal and state governmental regulations governing home health services; must demonstrate excellent nursing judgment and leadership ability; must possess a minimum of 2 years home health RN experience or one year of Home Health RN experience with a year of other management experience, with demonstrated ability to motivate and develop staff using team concepts, strong interpersonal/communication skills. Must have current BLS certification or be able to obtain within 60 days of employment and before first independent patient visit.

 

Must be able to read, write legibly, speak, and comprehend English.

Qualifications

Is able to lift 50 pounds, turn and position patients weighing over 200 pounds.  Ability to stand, walk, kneel, sit, crouch or stoop with good balance for extended periods of time.  Must be able to maneuver over a variety of uneven surfaces including fields, grass, ditches, gravel, concrete, wood, vinyl, or carpet. Must have tolerance in a variety of environments to include the presence of pets and infestation.

Good manual and finger dexterity.  Hearing and vision must be corrected to within normal range.  Must have excellent written and verbal communication skills. Must be licensed and insured to operate a motor vehicle and be able to travel to various locations.  Ability to work Monday-Friday 8 am to 5 pm holidays, weekends, and after hours as patient care and/or departmental business requires.

Physical presence onsite is essential.  Hearing and vision must be normal or corrected to within normal range.  Able to perform the duties with or without reasonable accommodation.
