Overview

Responsible for the evaluation, planning, implementation, treatment and documentation of care rendered to patients requiring physical therapy services. Clinical scope of services includes patient populations from neonate to geriatrics. Care, treatment and service provided to patients in their home.

Responsibilities

  1. Review, collect and evaluate relevant clinical data.
  2. Formulate an appropriate patient care plan based on assessment.
  3. Collaborates and coordinates patients care plans with patient/family and other care providers.
  4. Perform therapeutic procedures for clients as defined in department’s scope of service.
  5. Participates in patient/family teaching.
  6. Maintain clinical and professional competency based upon established standards of practice.
  7. Completes all documentation requirements in the established time frame.
  8. Responsible for monitoring all care delivered by assistants.
  9. Attends in service programs and departmental or hospital meetings.
  10. Shares experience by mentoring students, new employees and volunteers and presenting in-services.
  11. Maintains involvement in department and hospital activities.
  12. Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
  13. Performs compliance requirements as outlined in the Employee Handbook
  14. Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
  15. Requires use of electronic mail, time and attendance software, learning management software and intranet.
  16. Must adhere to all DCH Health System policies and procedures.
  17. All other duties as assigned.

Qualifications

Bachelor’s degree or Masters Degree in Physical Therapy.  Licensed as Physical Therapist. Maintains active status with license in good standing with the state board or licensing agency. Must have current BLS certification or be able to obtain within 60 days of employment and before first independent patient visit. Working knowledge of computer systems. Excellent coordination and communication skills. Detail oriented and able to work with minimal supervision. Licensed driver with automobile that is in good working order and insured.  Must be able to read, write legibly, speak and comprehend English.

 

WORKING CONDITIONS

Prolonged standing and walking. Assist in turning and positioning patients up to and beyond 200 lbs. Repeated bending, squatting, and stooping. Exposed to all patient elements. Must be able to reach overhead to retrieve and or replace objects. Must have tolerance in a variety of environments to include the presence of pets and infestation.

 Must have ability to communicate with others and handle crisis situations. Subject to stress and fatigue.

Physical presence onsite is essential.  Hearing and vision must be normal or corrected to within normal range.  Able to perform the duties with or without reasonable accommodation.
