Under the supervision of the Accounts Payable Supervisor performs a variety of functions to process vendor payments and other related jobs for the DCH Health System.
The RN Care Manager manages all aspects of the patient’s home health care from the start of care through discharge in collaboration with team visit nurses and other disciplines as determined by the needs of the patient. This position works 7 consecutive days and is off 7 consecutive days. He or she determines patient eligibility for care, completes most OASIS assessments, and develops evidenced-based individualized care plans following established professional standards of care, Agency policies, and State, Federal laws and regulations. The RN Care Manager is integral to optimal quality and process outcomes.
The intake specialist is responsible for obtaining accurate information for each home medical equipment referral/order received based on the payers’ guidelines and specified criteria. The intake specialist will utilize the information listed in the Medicare Supplier Manual, the Local Coverage of Determination, the Articles and the Program Integrity Manual to determine the appropriate data to be collected and to ensure that all coverage and billing criteria is met. The intake specialist will determine when an Advance Beneficiary Determination is needed and will discuss all payment criteria with the patient prior to delivery. Major responsibilities include management of all equipment referrals, obtaining physician orders and certificates of medical necessity, verifying insurance and coverage criteria, gathering medical documentation to demonstrate medical necessity and initiation of an Advance Beneficiary Notice when appropriate. The intake specialist will provide requested additional documentation needed by CMS and work well with the other staff.

The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines, providing skilled care, education, treatment, and emotional support to patients. Monitors and assesses the care provided while insuring the patient attains and maintains their highest level of functioning. Insures that the Physician and all members of the care team are aware of pertinent changes in the patient’s condition and progress toward goals. Collaborates with the Home Health RN Care Manager insuring that the established plan of care is appropriate and is implemented and/or modified based on the patient’s responses to treatment.
The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines, providing skilled care, education, treatment, and emotional support to patients. Consistently provides exceptional customer services with regard to providing timely, accurate information and problem resolution. Provides sound interpretation and application of Human Resources (HR) policies and maintains basic working knowledge of all supported areas of HR. Assists with administration and/or implementation of HR policies and processes as appropriate and supports functions for HR Service Center.
Organizes and assists with applicant tracking, interview and event scheduling and ensures an organized and efficient onboarding for new hires.
Responsible for maintaining DCH buildings, equipment and grounds.
He/she must have an enhanced knowledge in the area of sonography/ultrasound. They must have the specific expertise and skills necessary for maintaining a safe sonography/ultrasound environment daily. They must know basic anatomy, physiology and physics as it pertains to sonography/ultrasound patient care.
- The Care Manager works collaboratively with interdisciplinary and healthcare team members, both internal and external to the organization to facilitate patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve safe discharge planning along with desired clinical and financial outcomes. The Care Manager coordinates care and services through the acute care episode and across the continuum.

The goal for the IT Analyst – Help Desk (ITA-HD) is to achieve total end-user satisfaction by providing an exceptional customer service experience while resolving technical and software problems via telephone and email support. A person serving in this capacity helps end users navigate the best practices and standard DCH procedures for proper, secure usage of IT resources.
The ITA-HD serves as a key resource between DCH Health System departments, their contracted agencies and Information Technology. Creates and updates policies and procedures related to the use, management, and support of technology/applications.
Serves as a primary resource for technology/applications project planning, management and support with special focus on high-end solutions. Evaluates, recommends, configures, and installs technology to meet customers' needs. Works on complex implementation, development and support projects. The Laboratory Informatics Specialist is responsible to collaborate with laboratory leaders, patient care teams (Quality, Medication Safety, PI, etc.), as well as IT and other Informatics teams to develop laboratory system efficacy. The Laboratory Informatics Specialist will participate in organizational committees and serve as an expert Laboratory Informatics resource, providing guidance and perspective on the design and development of laboratory information systems to ensure that implementations support and enhance patient care, as well as the organization’s operational and strategic priorities. The Laboratory Informatics Specialist must be able to multi-task, perform all duties independently with strong prioritization skills and be self-directed in utilizing resources to meet objectives.
